Often people just start working without thinking about it beforehand - especially when they're self-employed. That's not necessarily a bad thing, but it can lead to problems. Personally, I set myself a phase plan with milestones, for example until a website goes live. After that, it continues in an iterative manner in weekly or monthly cycles.
